By participating in a virtual appointment, this action implies your consent to using telemedicine services and your understanding of the following:
- Health consultation will be provided via telemedicine. Although confidentiality and HIPAA compliance will be maintained to the best of Reisenauer Dermatology’s ability, there are potential risks to this technology, including interruptions and technical difficulties. I understand that my health care provider or I can discontinue the telemedicine consult/visit if it is felt that the videoconferencing connections are not adequate for the situation.
- I understand and consent to the fact that billing will occur according to my insurance policy and is subject to the rates and reimbursements of my policy for telemedicine services. If these telemedicine services are not covered by my insurance, I understand that I am responsible for payment as follows.
Live Video Teleconferencing: $60
- I understand that in the event that urgent in-person care is required, my provider may choose to schedule me for an in-office appointment.
